Complete the Coweeta Supply Order Form and submit it to the business office two weeks before delivery date is needed (the business office places orders on Thursday afternoons). Please also include a photocopied page of the catalogue from which you are ordering, or a printed page from the website from which you are ordering with the requested item circled. given to the business office and have permission from a PI to charge to a
